The Kaiser Foundation Hospitals, Panorama City Community Benefit Charitable Contributions Program recently awarded the Diabetes Education Department at Samuel Dixon Family Health Center, Inc. (SDFHC) with $5,000.00 in grant funds.
These funds are awarded in support of the Diabetes Education Program at SDFHC focused on providing accessible Diabetes Education and awareness for low income, uninsured and underinsured members of the community with pre-diabetes and diabetes.
SDFHC began offering weekly classes in English and Spanish in October of 2017 in response to the needs of this target population. In addition to education and awareness, the two hour group discussion covers DM2 pathogenesis, DM2 misconceptions, co-morbidities, nutrition education, reading food labels and the role of exercise.
A custom profile is prepared and distributed to each patient with a detailed discussion on the role and goal of the following measures: HgbA1c, blood pressure target, cholesterol/CVD risk, renal function/microalbuminuria, dental exams, yearly diabetic retinal screenings and yearly foot exams. The key to managing diabetes lay with patient involvement, education and empowerment.
SDFHC is honored to be awarded these funds to further our mission and serve those in the community and greatly appreciates the continued support from the Kaiser Foundation Hospitals.
